Application Procedures

Prime Health Foundation has two grant categories, annual grants and discretionary fund grants, with different requirements.

Annual Grants

Applications for annual grants are accepted once per year and must be submitted electronically by midnight, August 1, 2016. Decisions are typically finalized in December, and grants funded in the first quarter of the following year.

Annual grant amounts vary based upon the scope of the request and the funding amount available for that year.

Historically, the Foundation has favored giving smaller monetary grants to a larger number of organizations. Amounts typically range from $10,000 to $40,000, with a 2016 average of $25,200. In general, the Prime Health Foundation does not support capital campaigns.

Application Requirements for Annual Grants

Applications are due by midnight, August 1st by electronic submission via the link below. Verification of receipt will be sent to the email address of the contact person provided. Grant recipients will be expected to complete the midyear Grant Progress Report by midnight July 1st following receipt of the funds. Recipients will also be expected to complete a Grant Final Report by midnight January 31.

Discretionary Fund Grants

Each year, the board chair may approve small grants from the Discretionary Fund, typically ranging between $500 and $2,500. Discretionary Fund grants have been used to support health-related fairs, conferences and events, media or arts programs on health issues, and equipment and supplies.

Requests are accepted at any time and decisions are generally made within 60 days of receipt.
